Is UnitedHealth Stock Halal? Insurance and Healthcare Under Review

UnitedHealth combines conventional health insurance with care delivery, pharmacy benefits and health technology. Its financial ratios and business activities require separate Sharia analysis.

Time-sensitive screening snapshot: financial ratios and business mix can change after each filing. This is educational research, not a fatwa or investment advice. Verify the latest filing and your preferred Sharia standard before acting.

Quick Verdict

ZakatInvest currently classifies UnitedHealth Group (UNH) as doubtful. Its March 2026 total-assets financial ratios pass the examined methods, but conventional insurance is its dominant disclosed revenue source and raises gharar, maysir and riba concerns under many scholarly approaches. The filing does not provide the screened business-revenue numerator needed to turn that qualitative question into one universal percentage.

What UnitedHealth Does

UnitedHealth has two broad platforms. UnitedHealthcare provides employer, individual, Medicare, Medicaid and other health coverage. Optum provides care delivery, pharmacy-benefit services, health technology, analytics and advisory services. Treating every dollar identically would hide important differences between an insurance contract, a medical service, a prescription product and administrative technology.

For the quarter ended March 31, 2026, UnitedHealth reported $87.561 billion of premium revenue, $13.250 billion of product revenue, $9.779 billion of service revenue and $1.131 billion of investment and other income. Premiums were 78.37% of the $111.721 billion consolidated total, but that share is not automatically a prohibited-revenue percentage under every Sharia methodology.

Why Conventional Insurance Is Disputed

Many scholars object to conventional commercial insurance because the exchange can contain substantial uncertainty about payment and benefit, a wager-like transfer of risk, and interest-bearing investment of premiums. Takaful seeks cooperative risk sharing instead. Other rulings can distinguish compulsory or employer coverage, public need, jurisdiction, contract design and the absence of a viable cooperative alternative. This article therefore identifies the concern without presenting one school's treatment as unanimous.

UnitedHealth's beneficial healthcare services do not erase the contract-level insurance question, and the insurance question does not make every Optum activity inherently prohibited. A defensible verdict has to preserve both facts.

Business-activity disclosure

UnitedHealth combines conventional health insurance through UnitedHealthcare with care delivery, pharmacy-benefit, health-technology and advisory services through Optum. Healthcare that preserves life and health is beneficial in itself. Conventional commercial insurance is disputed or rejected by many scholars because of gharar, maysir and riba concerns, while rulings can distinguish necessity, cooperative risk sharing, regulation, employer coverage and takaful alternatives.

Limitation: Premium revenue was 78.37% of consolidated quarterly revenue, but that is not automatically a prohibited-revenue percentage under every methodology. The filing does not disaggregate revenue according to a chosen scholarly treatment of conventional insurance, government programs, care delivery, pharmacy benefits, interest, investments, or specific products and practices.

Purification

Treating the full 1,131 investment-and-other-income line as potentially non-compliant gives a conservative 1.01% upper bound of quarterly revenue. Because that line is mixed and screened operating revenue is unavailable, this record does not prescribe a complete fixed purification percentage.

Inputs, assumptions and primary sources
  • Interest-bearing debt is the sum of 6,477 of short-term borrowings and current maturities plus 71,440 of long-term debt.
  • Cash and cash equivalents use the reported 28,001 balance.
  • Interest-bearing securities include 50,106 of available-for-sale debt securities and 474 carrying value of held-to-maturity debt securities. Marketable equity securities are excluded from this input.
  • The receivables input deliberately combines 26,587 of accounts receivable with 24,588 of other current receivables as a conservative upper bound.
  • Revenue and the 1,131 investment-and-other-income line use the same three-month period ended March 31, 2026. The entire mixed income line is treated as potentially non-compliant rather than assuming an undisclosed interest amount.
  • UnitedHealth reported 87,561 of premium revenue, 13,250 of product revenue and 9,779 of service revenue, but the filing does not provide a Sharia-screened numerator for conventional insurance, pharmacy-benefit, care-delivery, investment, or other activity.

This is a reproducible financial screen, not a fatwa. A failed total-assets screen can coexist with a pass under a market-cap methodology. ZakatInvest keeps the qualitative assessment separate so numerical thresholds do not erase material context.

What the Numbers Show

Debt is 24.92% of total assets. Cash plus identified debt securities is 25.13%, and a deliberately conservative measure that combines accounts receivable, other current receivables and cash is 25.32%. Treating all investment and other income as potentially non-compliant produces a 1.01% upper bound. Those examined financial ratios pass.

They do not finish the business screen. UnitedHealth does not disaggregate revenue according to a chosen scholarly treatment of conventional insurance, public programs, pharmacy benefits, care delivery, investments or specific practices. Proper market-cap methods also require historical average denominators, which ZakatInvest does not approximate from a current share price.

Broader Qualitative Review

Healthcare supports preservation of life and can deliver substantial public benefit. Investors should also examine claims administration, coverage denials, prior authorization, affordability and provider reimbursement; pharmacy-benefit pricing, rebates, formularies and access; provider consolidation and competition; government-program compliance; patient-data privacy and cybersecurity; and the continuing effects of the Change Healthcare disruption. These are stewardship and justice concerns, not substitutes for the formal ratios.

Purification

The reported $1.131 billion line combines investment income with other items. Treating all of it as potentially non-compliant gives a conservative 1.01% upper bound, not a precise interest figure. Because screened operating revenue is also unavailable, ZakatInvest does not prescribe a complete fixed purification percentage for UNH.

Bottom Line

UNH remains doubtful in ZakatInvest's current classification, while its examined total-assets financial screens pass. The unresolved issue is the business-activity treatment of conventional insurance and related mixed activities, not a failure of the current debt, liquidity, receivables or income ratios. This is a documented screening classification, not an investment recommendation or fatwa; investors seeking a binding ruling should apply their chosen methodology with a qualified Sharia adviser.
